The175-bed facilityoffers the infrastructure, expertise, compassion, support and care toholisticallytreat and rehabilitate children through amultidisciplinaryapproach of24-hour nursing care andrehabilitation. In the Western Cape, with a population of 6,2 million, there is only one institution that provides intermediate facility-based care for children with chronic or life-limiting conditions. The patient profile of admissions in recent years has changed and requires intensive and holistic care plans with the full spectrum of supporting rehabilitation therapy and nursing services. The needs of the facility increase year on year and we have a dedicated fundraising office for resource mobilisation and development and in-kind donations. We believe that no child should be left behind because of the circumstances they were born into or their need for ongoingmedical care, rehabilitation and psycho-social supportafter being discharged from hospital. Our holistic approach to further support a patient’s recovery process also includes nutritional, psycho-social, logistical, educational and spiritual support. Parental empowerment and education helps to improve medication adherence and understanding of the patient’s medical condition when they are discharged. Located in Montana, Cape Town, St Joseph’s is a non-profit paediatric intermediate care facility providing post-acute palliative, restorative, rehabilitation and respite care. We serve severely vulnerable communities and act as a bridge between acute care in hospital and the patient going home.

St Joseph’s Home for Chronically Ill Children is a registered non-profit organization which has been a pioneer in the field of paediatric intermediate health care in South Africa. We provide free transitional care for chronically ill children from disadvantaged families, from hospital to home through a holistic health and wellness programme. Patients are referred from State Hospitals to continue with post-acute, restorative, palliative and rehabilitative care. It is imperative that children are not confined to a hospital environment longer than necessary.
Smaller groupings of beds are created – each with a thickened wall to accommodate a sense of one’s own place to put things. Roofs were lifted so that if a child has to remain in his bed, he can see the clouds or stars. These high level windows also assist with naturally ventilating the wards as well as ensuring a high level of light. Thick walled bays were added with low ledges, scaled around the child and with large sliding windows that open onto garden. Medically stable children who need post-acute care are referred by public hospitals and we work closely with the referral team to optimise the recovery process while the patient is in our care. Therefore, without funding from corporates, trusts and foundations and individuals we would not be able to continue delivering quality healthcare to medically fragile children. Up to 500 children a year are looked after by St Joseph’s Home. A well-loved institution in Cape Town, it has provided intermediate, facility-based care for children with chronic or life-limiting conditions for the last 85 years.
The main intention of the project is to create a safe and happy ‘container’ for the children, and was made possible by a private donation. Although every effort is made to implement cost-saving measures, we are in need of funding to ensure our patients’ recovery process is secure and they reach their rehabilitation milestones. On average a child remains in our care for 6-months and we rely on donations to ensure their stay with us is as comfortable as possible.
